DreemTeem HomeCare LLC is seeking a compassionate, organized Care Team Manager to lead our caregiving team. In this leadership role you will coordinate schedules, support and mentor caregivers, communicate with clients and families, and help uphold the high standard of care our clients deserve.

Monday-Friday, flexible schedule

3+ years in home care or senior care

Key Responsibilities:

Supervise, support and mentor the caregiving team

Coordinate caregiver schedules and client assignments

Serve as the main point of contact for clients and families

Conduct caregiver check-ins, training and performance reviews

Ensure care plans are followed and documentation is complete

Help resolve client concerns quickly and professionally

Requirements:

3+ years of experience in home care or senior care

Prior supervisory or team lead experience preferred

Strong communication and organizational skills

Compassionate, dependable and detail-oriented

Valid driver's license and reliable transportation

Key Responsibilities:

Assist clients with personal care activities (bathing, grooming, dressing)

Provide companionship and emotional support

Help with mobility and transfers

Prepare meals and assist with feeding as needed

Medication reminders

Light housekeeping and laundry

Transportation to appointments and errands

Document and report changes in client condition

Requirements:

Compassionate, patient, and reliable

Valid driver's license and reliable transportation

Ability to pass a background check

Strong communication skills

Ability to lift and assist clients with mobility

CNA certification required
